How It Works:

1. Complete the Free Online Assessment – Click below to start your free online

assessment for gym memberships and personal training fees.

2. Get Approved in Minutes – Current and prospective members take the brief telehealth survey, paying $20 at the end.

3. Submit for Reimbursement – If approved (~99% of those who reach payment screen), members receive a Letter of Medical Necessity (LMN) along with instructions on how to submit expenses for reimbursement. (Takes <6 hours to receive).

The LMN is valid for 12 months, so members can submit membership and fitness program receipts for reimbursement from their pre-tax HSA/FSA accounts during that time.
